In her autobiography, Nichols said she had an affair with Gene Roddenberry, the man who created Star Trek. Foster Johnson (1917–1981), a dancer, was twice married to Nichols. They married in 1951 and divorced the following year. As far as we know, Johnson and Nichols only have one child together, Kyle. She married Duke Mondy in 1968 and divorced him two years later.

An announcement made in 2015 by Nichols’ booking agency stated that she had been hospitalized after suffering a minor stroke at her Los Angeles home that year. Her small stroke was discovered during an MRI scan in 2015, and as a result, she entered inpatient rehabilitation. After being diagnosed with dementia in 2018, Nichols has decided to retire. It was at this point that she announced that she would no longer attend conventions.
